Base pay range
$28.05/hr - $42.10/hr

What To Expect (Job Responsibilities)
- Coordinate healthcare services for patients in the Advanced Care at Home program, including triaging clinical inquiries and assisting with patient monitoring
- Facilitate effective communication between patients, internal teams, and external service providers
- Manage the referral process for appropriate departments, ensuring accurate data entry and compliance with insurance requirements
- Conduct patient outreach to provide support, education, and disease management instruction
- Monitor and generate reports for effective caseload management
What Is Required (Qualifications)
- Licensed Practical Nurse (LPN) license issued by the state of Illinois
- Completion of an accredited or approved program in Practical Nursing
- Typically requires 3 years of experience in an LPN role
- Excellent telephone, interpersonal, and written communication skills
- Strong knowledge of Medicare rules and regulations
