Facilitator SNAP

About the position

Implements the SNAP (Stop Now And Plan) Program at assigned locations. This individual is responsible for facilitating the SNAP curriculum with children and families, supporting positive behavioral change through evidence-based interventions, and collaborating with program staff, schools, families, and community partners to ensure successful program implementation.

Responsibilities

  • Successfully completes all required SNAP trainings and maintains program fidelity in accordance with SNAP standards.
  • Conducts participant intakes, screenings, and assessments, as assigned.
  • Maintains regular contact and ongoing engagement with children and families throughout the duration of the SNAP program.
  • Builds positive, supportive relationship with participants and caregivers to encourage engagement and successful completion of the program.
  • Works collaboratively with the SNAP Coordinator, SNAP team members, school personnel, referral sources, and community partners to coordinate services and support participant success.
  • Assists with participants recruitment, enrollment, orientation, and retention efforts.
  • Maintains accurate and timely documentation, including attendance, case notes, intake documentation, assessments, communication logs, and other required program records.
  • Keeps accurate records and submits reports and documentation within established timeframes.
  • Participates in staff meetings, case consultations, supervision, required trainings, and program planning activities.
  • Assists with communication, material preparation, and coordination of program activities as needed.
  • Maintains the highest level of professionalism, confidentiality, and participant safety in all programming.
  • Provides referrals and connects families with appropriate community resources, as needed.
  • Maintains regular communication with families, schools, referral agencies, and appropriate support staff to ensure quality service delivery.
  • Assists with program evaluation activities and data collection, as requested.
  • Other duties as required.
  • Adheres to all agency policies, laws, regulations, and code of ethics and confidentiality as outlined by federal and state laws, and agency policies and procedures.

Requirements

  • Bachelor’s Degree in Social Work, Psychology, Counseling, Criminal Justice, Education, Human Services, or a related field required; equivalent combination of education and experience may be considered at the discretion of the agency.
  • Strong verbal, written, organizational, and interpersonal communication skills.
  • Culturally responsive and sensitive to the diverse needs of the community.
  • Must successfully obtain Criminal History Clearance from the Pennsylvania State Police and Child Abuse Clearance from the Pennsylvania Department of Human Services and Fingerprint Based FBI Clearance.
  • Must not be excluded from participation in Medicare, Medicaid, or any other federal health care program.
  • Ability to transport oneself as program needs dictate and maintain a valid driver’s license.
  • Ability to travel throughout the service area to schools, homes, community sites, and agency locations.
  • Ability to work independently while functioning effectively as part of a multidisciplinary team.
  • Ability to work flexible hours, including evenings, to accommodate participant and family schedules.

Nice-to-haves

  • Experience working with children, adolescents, and families, preferred.
  • Experience facilitating groups, providing behavioral interventions, or working with school or community-based settings preferred.
