Care Navigator (Remote) — Bilingual Required
<p>A mission-driven organization working at the intersection of food, healthcare, and technology is hiring <strong>Care Navigators</strong> to support New York's <strong>Medicaid Waiver</strong> program.</p><p><br></p><p>In this role, you'll be the first point of contact for Medicaid members — screening for eligibility, assessing <strong>Health-Related Social Needs (HRSN)</strong>, and connecting members to the services and community partners that can help them live healthier lives. If you're organized, compassionate, tech-savvy, and thrive in a fast-paced virtual contact center, we'd love to hear from you.</p><p><br></p><p><strong>The Details</strong></p><p>Fully remote</p><p>Monday–Friday, 9a-6p + 2 Saturdays per month</p><p>$25/hour</p><p><strong>Bilingual required — Spanish or Mandarin</strong></p><p><strong>*must reside within 60 miles of NYC</strong></p><p><br></p><p><strong>What You'll Do</strong></p><ul><li>Manage inbound and outbound member communications by phone and email using Five9 and partner platforms</li><li>Screen potential members, assess eligibility, and navigate them to the right service providers based on their needs</li><li>Track and follow up on referrals to ensure services are delivered</li><li>Maintain accurate documentation across program and internal systems</li><li>Support members with program questions</li><li>Comply with Medicaid, HIPAA, and NYS DOH requirements</li></ul><p><br></p><p><strong>What We're Looking For</strong></p><ul><li>Associate degree required</li><li>3+ years in customer service, call centers, social services, care navigation, or a related field</li><li>Experience in a virtual contact center (Five9 preferred)</li><li>Strong communication and member-engagement skills</li><li>Comfort managing high-volume interactions in a compliance-driven environment</li></ul><p><br></p><p><strong>Nice to Have</strong></p><ul><li>Experience with Medicaid, Social Care Networks (SCNs), or HRSN programs</li><li>Background in healthcare, care coordination, patient support, or community-based services</li></ul><p><br></p>
